A. The Insurance Commissioner shall collect in advance the following fees: 1. For filing appointment of Insurance Commissioner as agent for service of process.........................................$10.00 4. There shall be assessed an annual fee of Five Hundred Dollars ($500.00) payable by each insurer, health maintenance organization, fraternal benefit society, hospital service and medical indemnity corporation, or charitable and benevolent corporation licensed to do business in this state, or United States surplus lines insurance companies approved to do business in this state, to pay for the filing, processing, and reviewing of financial statements by personnel of the Office of the State Insurance Commissioner.
