Oklahoma Code § 26-7-115

Title 26. Elections: Absentee ballot affidavit

If a registered voter has requested an absentee ballot, he shall
be required by the judge to sign an affidavit swearing or affirming
that he has not cast such absentee ballot and is entitled to vote in
person.
