Oklahoma Code § 12-1804

Title 12. Civil Procedure: Written consent to dispute resolution proceedings

A.  Prior to commencement of any dispute resolution proceedings,
the disputing parties shall enter into a written consent which
specifies the method by which the parties shall attempt to resolve
the issues in dispute.
B.  The written consent shall be in a form prescribed by the
Administrative Director of the Courts and shall include the
following:
1.  The rights and obligations of all parties pursuant to the
provisions of the Dispute Resolution Act; and
2.  The confidentiality of the proceedings.
C.  If the parties agree to have the resolution reduced to
written form, a copy shall be provided to the parties.
