Oklahoma Code § 10A-2-5-211

Title 10A. Children And Juvenile Code: Commitment to Department of Corrections - Judgment

When committing a person who is, or has been, certified as a
youthful offender and is certified eligible for the imposition as an
adult sentence pursuant to Section 2-5-208 of this title, or
certified as a youthful offender and is being transferred to the
Department of Corrections for custody or supervision pursuant to
Section 2-5-210 of this title, or sentenced as an adult after
previously being certified as a youthful offender, the judgment and

sentence shall clearly identify such person as a youthful offender,
or previous youthful offender, and detail the history of the
applications of the Youthful Offender Act to such person that
resulted in the current commitment to the Department of Corrections.
