Effective: September 29, 2013 Latest Legislation: House Bill 59 - 130th General Assembly There is hereby established the patient centered medical home program in the department of health. The patient centered medical home model of care is an advanced model of primary care in which care teams attend to the multifaceted needs of patients, providing whole person comprehensive and coordinated patient centered care.
