Effective: March 20, 2014 Latest Legislation: Senate Bill 23 - 130th General Assembly The department of health shall include on its web site information about biological parent's name redaction request forms. All of the following information shall be provided: (A) The purpose of the form; (B) The procedures to be followed and requirements to be met for the department to accept the form; (C) The date when biological parents may begin to file the form with the department; (D) The date when the form may no longer be filed with the department; (E) The procedures to be followed and requirements to be met for having the form removed from an adopted person's adoption file; (F) Any other information the department considers necessary.
