Physicians licensure with the board

1. An applicant shall file with the executive director of the board a completed application 
and shall pay to the executive director the application fee. No person may engage in 
the practice of medicine in this state without a current license issued by the board.
2. Each licensee shall maintain a permanent electronic mail or mailing address with the 
board to which all communications from the board to the licensee will be sent. A 
licensee who changes the individual's electronic mail or mailing address shall notify 
the board in writing of the new contact information within sixty days.
3. If a licensee fails to notify the board in writing of the changes as required by this 
section after sixty days, the board may impose upon the licensee a fee not to exceed 
one hundred dollars and may initiate disciplinary action against the licensee.
