Application for operator's license

1. An applicant for an operator's license must be made upon a form furnished by the 
director.
2. An applicant must state on the application the full name, date of birth, sex, social 
security number, unless the applicant is a nonimmigrant who is not eligible for a social 
security number or the applicant provides an affidavit stating the applicant was not 
assigned a social security number, residence and mailing address, and provide a brief 
description of the applicant. By signing the application the applicant is deemed to have 
certified that all information contained on the application is true and correct. The 
application must be accompanied by the fee listed in section 39-06-49. The application 
must contain any other information as the director may require to improve identity 
security. The director may require an applicant to provide a social security card and 
proof of residence address.
3. If an application is received from an individual previously licensed in another 
jurisdiction, the director may request a copy of the driver's record from the other 
jurisdiction. A copy of another jurisdiction's driving record becomes a part of the driving 
record in this state with the same force and effect as though entered on the driving 
record in this state in the original instance.
