North Dakota Code § 10-33-16

Articles of amendment

When an amendment has been adopted, articles of amendment must be prepared that 
contain:
1. The name of the corporation.
2. The amendment adopted.
3. The date of the adoption of the amendment pursuant to this chapter.
4. If the amendment restates the articles in their entirety, a statement that the restated 
articles supersede the original articles and all amendments to them.
5. A statement that the amendment has been adopted pursuant to this chapter.
