(a) A person who holds a license or registration with the Board shall notify the Board in writing of the address of the place or places where he or she engages or intends to engage in the practice of speech and language pathology or audiology. (b) The Board shall keep a record of the places of practice of licensees and registered assistants. (c) Any notice required to be given by the Board to a licensee or registered assistant may be given by mailing it to him or her at the address of the last place of practice of which he or she has notified the Board.
