A proposition to approve an alteration shall be printed on the ballot in substantially the following form: "Shall the structure of the board of commissioners be altered? (Describe the effect of the alteration.) [] YES [] NO" The ballot shall be separate from other ballots used at the election. If a majority of the votes cast on the proposition are in the affirmative, the plan contained in the resolution shall be put into effect as provided in this Part. If a majority of the votes cast are in the negative, the resolution and the plan contained therein are void.
