§ 29. Intake forms for admission and residency. 1. The department, in\ncooperation with the office of temporary and disability assistance and\nany other state department, office, division or agency the department\ndeems necessary, shall require that all intake forms for admission or\nresidency to any temporary shelter that is reimbursed from state or\nstate-administered grants or funds shall ask an applicant: "Have you or\nanyone in your household ever been in the United States military?". Each\nsocial services district or social services district's designee shall in\nwriting advise all individuals applying for temporary housing assistance\nand identifying themselves as having been in the United States military\nthat the department of veterans' services and local veterans' service\nagencies established pursuant to section fourteen of this article\nprovide assistance to veterans regarding benefits available under\nfederal and state law. Such written information shall include the name,\naddress and telephone number of the New York state department of\nveterans' services, the nearest department of veterans' services office,\nthe nearest county or city veterans' service agency and the nearest\naccredited veterans' service officer. Each social services district or\nsocial services district's designee, with the permission of such\nindividual's identifying themself as a veteran, shall transmit such\nveteran's status information to the department of veterans' services.\n 2. The department, in cooperation with the office of temporary and\ndisability assistance and any other state department, office, division\nor agency the department deems necessary, shall encourage all other\ntemporary shelter providers to share information to increase veteran\naccess to benefits by:\n (a) providing information on the department website including:\n (i) potential questions for inclusion on intake forms including, but\nnot limited to: "Have you or anyone in your household ever been in the\nUnited States military?";\n (ii) advising such providers that all individuals identifying\nthemselves as having been in the United States military that the\ndivision and local veterans' service agencies provide assistance to\nveterans regarding benefits available under federal and state law; and\n (iii) the address and telephone number of the department, county and\ncity veterans' service agencies and accredited veterans' service\nofficers; and\n (b) facilitating the transmission of such veteran's status\ninformation, with the permission of individuals identifying themselves\nas a veteran, to the department.\n
