§ 193. Form and contents of license; display of license. 1. Each\nlicense to conduct games of chance shall be in such form as shall be\nprescribed in the rules and regulations promulgated by the board, and\nshall contain a statement of the name and address of the licensee, of\nthe names and addresses of the members of the licensee under whom the\ngames will be managed, of the place or places where and the date or\ndates and time or times when such games are to be conducted and of the\npurposes to which the entire net proceeds of such games are to be\ndevoted; the name of each single type of game to be conducted under the\nlicense and the number of merchandise wheels and raffles, if any, to be\noperated; and any other information which may be required by said rules\nand regulations to be contained therein, and each license issued for the\nconduct of any games shall be conspicuously displayed at the place where\nsame is to be conducted at all times during the conduct thereof.\n  2. Each license to lease premises for conducting games of chance will\nbe in such form as shall be prescribed in the rules and regulations of\nthe board and shall contain a statement of the name and address of the\nlicensee and the address of the leased premises, the amount of\npermissible rent and any information which may be required by said rules\nand regulations to be contained therein, and each such license shall be\nconspicuously displayed upon such premises at all times during the\nconduct of games of chance.\n
