§ 358. Reporting. 1. Each participant must submit a performance report\nannually, in such form as the commissioner may require, within thirty\ndays of the end of their taxable year.\n  2. The commissioner shall prepare on a quarterly basis a program\nreport for posting on the department's website. The first report will be\ndue June thirtieth, two thousand eleven, and every three months\nthereafter.  Such report shall include, but not be limited to, the\nfollowing: number of applicants; number of participants approved; names\nof participants; total amount of benefits certified; benefits received\nper participant; total number of net new jobs created; number of net new\njobs created per participant; aggregate new investment in the state; new\ninvestment per participant; and such other information as the\ncommissioner determines.\n  3. The commissioner shall prepare on a quarterly basis information\nrelated to the utilization of the excelsior child care services tax\ncredit component for inclusion in the quarterly excelsior jobs program\ntax credit reports required pursuant to subdivision two of this section.\nSuch information shall include, but need not be limited to the\nfollowing: number of applicants; number of participants approved; total\nnet new child care services expenditures certified; total amount of\nbenefits certified; benefits received per participant. On an annual\nbasis, businesses participating in the excelsior child care services\ncredit shall report to the department on the number of employees\nparticipating in child care services supported by the credit.\n
