New Hampshire Code § 5-C:60

Application for and Preparation of a Certified Copy of a Divorce, Legal Separation, or Civil Annulment

I. An applicant for a copy of a divorce, legal separation, or civil annulment certificate shall provide the following information, in writing, to the division: (a) The husband's first and last name. (b) The wife's first and maiden name. (c) The date of decree. (d) The place of decree by county. (e) The purpose for which the certificate is requested, in order to establish direct and tangible interest in the record. (f) The signature of the applicant. (g) The applicant's relationship to the husband or wife. (h) The applicant's name, address, and telephone number. II. If the applicant is determined by the state registrar to have a direct and tangible interest in the record, a certified copy of a divorce, legal separation, or civil annulment record shall be issued by the division, for any year of occurrence.
