I. In response to board inquiries relevant to a licensee's status or practice of nursing or nursing-related activities, each licensee shall provide complete and truthful information. II. Each licensee shall notify the board if a license is lost or stolen. III. Each licensee shall notify the board of a change of name or address. IV. Each licensee shall report to the board those acts or omissions which are violations of this chapter or grounds for disciplinary action.
