1. Each applicant for a license as a barber or an apprentice must file an application verified by him or her for an examination before the Board. 2. The application must be in a form prescribed by the Board. 3. Each application must be accompanied by the fees prescribed by subsection 4. 4. The Board shall annually fix the examination fees, which must not be more than $100. 5. Each applicant must, at the time of filing the application, file a certificate signed by a licensed physician certifying that the applicant is free from tuberculosis and other communicable diseases. 6. Each applicant must submit all information required to complete the application.
