Certification regarding other wholesalers

A person who is licensed to engage in
wholesale distribution pursuant to this chapter shall certify a claim by
another wholesaler that the wholesaler is an authorized distributor of record
from whom the licensee purchases a prescription drug. Such certification
includes a statement signed by a representative of the wholesaler certifying
the claim that the wholesaler is an authorized distributor of record for a
specified manufacturer and:
1. A copy of the written agreement
currently in effect with the manufacturer;
2. A copy of a letter from the
manufacturer endorsing the wholesaler as an authorized distributor of record;
3. Copies of applicable invoices from the
manufacturer demonstrating the purchase by the wholesaler of at least 1,000
sales units of prescription drugs from the manufacturer within the 12 months
immediately preceding the current month;
4. Copies of applicable invoices from the
manufacturer from each of the previous 12 months;
5. Copies of applicable invoices from the
manufacturer specific to the given transaction; or
6. Verification from the manufacturers
website that the wholesaler is an authorized distributor of record.
