1. A record of each refill of any prescription for a controlled substance or dangerous drug or any authorization to refill such a prescription must be kept: (a) On the back of the original prescription; (b) In a bound book or separate file; or (c) In an electronic record that is readily retrievable. 2. The record must include: (a) The date of each refill or authorization; (b) The number of dosage units; and (c) The signature or initials of the pharmacist who refilled the prescription or obtained the authorization to refill.
