Nevada Code § 639.190

Issuance of certificate of registration as pharmacist after forfeiture

If a certificate of
registration as a pharmacist is forfeited by a person as provided in NRS 639.180 , the Board may, within 5 years
thereafter, issue a certificate of registration to the person if the Board
determines that the person:
1. Has not committed any act listed in NRS 639.210 other than the failure to renew
the certificate by not submitting the application for renewal or the fee for
renewal; and
2. Is capable and qualified by education
or experience, or both, to practice the profession of pharmacy in this State.
