Requirements for prescriptions for and initial fitting of contact lenses

1. The form for any prescription which is
for a contact lens must set forth the expiration date of the prescription, the
number of contact lenses approved for the patient and such other information as
is necessary for the prescription to be filled properly. A prescription for
spectacle lenses must not be construed to be approved for or converted to a
prescription for contact lenses unless contact lenses are expressly approved in
writing on the prescription by the prescribing optometrist.
2. The initial fitting of a contact lens
must be performed by an ophthalmologist or optometrist licensed in this State.
3. As used in this section, initial
fitting means measuring the health, integrity and refractive error of the eye
to determine whether contact lenses are appropriate for the patient.
