Registration: Application; expiration and renewal; approval of late application for renewal; notification to Board of change of information; administrative fine for failure to provide such notification

1. To obtain a registration as a business
entity that provides chiropractic services, a business entity must submit to
the Board an application in the form prescribed by the Board. The application
must include:
(a) The name, address and telephone number of the
business entity;
(b) The name of any officer or director of the
business entity; and
(c) The name of any chiropractic physician who is
responsible for providing or supervising the provision of chiropractic services
in any facility owned or operated by the business entity.
2. A registration as a business entity
that provides chiropractic services expires on June 1 of each year and may be
renewed by submitting to the Board before the expiration of the registration an
application for renewal in the form prescribed by the Board.
3. The Board may approve a late
application for renewal.
4. A business entity shall notify the
Board in writing within 30 days after any change to the information described
in subsection 1.
5. The Board shall impose an
administrative fine in an amount prescribed by regulation of the Board against
a registrant that does not comply with the requirements of subsection 4.
