Genetic information includes, without limitation, information concerning: 1. A genetic test performed on a person; 2. A genetic test performed on the family members of a person; 3. A disease or condition that occurs or has occurred in the family members of a person; or 4. Genetic counseling or genetic education services provided to a person.
