Requirements of contract for purchase of services; copy of contract must be retained by organization

1. A contract between a buyer and an
organization for the purchase of the services of the organization:
(a) Must be in writing;
(b) Must be signed by the buyer;
(c) Must be dated; and
(d) Must clearly indicate above the signature
line that the buyer may cancel the contract within 5 days after its execution
by giving written notice to the organization of his or her intent to cancel the
contract. If the notice is mailed, it must be postmarked not later than 5 days
after the execution of the contract.
2. A copy of each contract executed by a
buyer and an organization must be retained by the organization for not less
than 2 years.
