Nevada Code § 449.443

Application for permit; fee; inspection by Division; term of permit

1. An office of a physician or a facility
that provides health care, other than a medical facility, desiring a permit
pursuant to NRS 449.435 to 449.448 , inclusive, must submit to the
Division, on a form prescribed by the Division and accompanied by the
appropriate fee, an application for a permit.
2. Before issuing a permit, the Division
shall conduct an on-site inspection pursuant to NRS 449.446 of each office of a physician
or facility that applies for a permit.
3. Upon receipt of an application and the
appropriate fee, the Division may, after conducting an inspection pursuant to NRS 449.446 , issue a permit.
4. A permit expires 1 year after the date
of issuance and is renewable pursuant to NRS
449.444 .
