Duties of hospital and licensed entity that provides care for patient upon discharge to ensure dietary needs of patient are met

1. If a patient will be released from a
hospital to his or her residence or a rehabilitation center and a dietitian is
assigned to a team of persons formed by the hospital to care for the patient
while the patient rehabilitates, the hospital shall ensure that the patient or
the person with primary responsibility for the care of the patient meets or
knows how to contact the dietitian.
2. If a patient is released by a hospital
to his or her residence to receive care from an entity licensed pursuant to
this chapter that provides care to the patient in his or her residence, the
entity shall consult with a dietitian, as appropriate, to ensure that the
patient or the person with primary responsibility for the care of the patient
understands the dietary needs of the patient.
