1. The Board shall prescribe, and the State Registrar shall furnish in sufficient numbers to each county clerk for distribution, a form for the reporting of divorces and annulments of marriage. 2. The information required by such form must be limited to: (a) The names of the parties; (b) The court and county in which the decree is granted; and (c) The date of the decree.
