Nevada Code § 340.050

Institution of proceedings: Filing and form of petition

A proceeding may be instituted under this
chapter by the filing of a petition which shall be sufficient if it sets forth:
1. The name of the petitioner.
2. A description of the property,
sufficient for the identification thereof, to which there may be attached a
plat or map thereof.
3. A statement that the acquisition of
such property by the petitioner is necessary for a public works project and a
brief general description of the public works project.
4. A statement that the proceedings are
being instituted under this chapter.
5. A suitable prayer for relief.
