The secretary of state may prescribe and furnish on request forms for all documents required or permitted to be filed by this chapter. The use of the following forms is mandatory: (1) A foreign corporation's application for a certificate of authority to do business in this state; (2) A foreign corporation's application for a certificate of withdrawal; (3) A corporation's corporate registration report.
