Each applicant for an examination shall: (1) make application to the Board of Barber Examiners on blank forms prepared and furnished by it, the application to contain proof under the applicant's oath of the particular qualifications and identity of the applicant; (2) provide all documentation required in support of the application; (3) pay to the board the required fee; and (4) upon acceptance of the notarized application present a corresponding government-issued photo identification when the applicant appears for examination.
