Loss, Destruction, or Mutilation of Registration, Certificate, or Trainee Permit; Replacement.

Sec. 19.
In the event of loss, destruction, or mutilation of a registration, certificate, or trainee permit, the person to which it was issued may obtain a replacement by furnishing satisfactory proof of the loss, destruction, or mutilation and paying the fee required under section 30. An application for a replacement registration, certificate, or trainee permit shall include all of the following information:
(a) The name and address of the applicant.
(b) An explanation of the loss, destruction, or mutilation of the original registration, certificate, or permit.
