(a) Each application for a commercial driver's license or commercial driver's
instructional permit shall be made on the form the Administration requires.
(b) In addition to the information specified by § 16-106 of this title, each
application shall contain:
(1) The applicant's Social Security number;
(2) A consent to release a complete driving record to any employer or
prospective employer;
(3) Certifications and information required by federal regulation;
and
(4) Any other pertinent information that the Administration
requires.
(c) Under penalty of perjury, the applicant shall sign the application and
certify that the statements made are true and correct to the best of his knowledge,
information, and belief.
(d) When a licensee changes the licensee's name, mailing address, or
residence, an application for a corrected license shall be submitted to the
Administration within 30 days.
(e) Within 30 days after establishing a residence in this State, the holder of
a commercial driver's license shall apply for a commercial driver's license in this
State.
