Section TR-15-107

If a license issued under this title is lost, stolen, mutilated, destroyed, or
becomes illegible, the Administration may issue a duplicate license on application
and payment of a fee established by the Administration. Before the Administration
issues a duplicate, it may require the licensee to furnish satisfactory proof of the loss,
theft, mutilation, destruction, or illegibility. When the Administration issues the
duplicate, the license previously issued is void.
