(a) The Board shall reinstate the license of an individual who has failed to renew a mortician license or funeral director license for any reason if the individual: (1) Requests that the Board reinstate the license; (2) Meets the appropriate renewal requirements of this subtitle; (3) Pays to the Board a reinstatement fee set by the Board; (4) Submits to the Board an affidavit stating that the individual did not practice mortuary science in this State while the license was expired; and (5) Applies to the Board for reinstatement of the license within 5 years after the license expires. (b) The Board may not reinstate the license of a mortician or funeral director who fails to apply for reinstatement of the license within 5 years after the license expires unless the mortician or funeral director meets the terms and conditions established by the Board.
