(a) Each licensee shall keep a license and collaboration agreement for inspection at the primary place of business of the licensee. (b) (1) A licensee shall provide the Board written notice of any change of name or address within 30 days after the change. (2) A licensee who fails to comply with this subsection is subject to an administrative penalty of $100. (c) The Board may audit and review collaboration agreements kept by the licensee at the primary place of business of the licensee at any time. (d) A physician assistant who fails to produce a collaboration agreement to the Board on request is subject to an administrative penalty as established in regulations.
