(a) To apply for a license, an applicant shall submit an application to the
Department on the form that the Department requires.

(b) The application form shall include:
(1) The applicant's name and address;
(2) The location of the food establishment for which application is
made;
(3) The type of food establishment that the applicant proposes to
operate; and
(4) Any other information the Department requires.
