(a) An applicant for a license shall submit an application to the Secretary on the form that the Secretary requires. (b) An application for a license to operate a forensic laboratory shall include: (1) The name of the operator or owner; (2) The tests or examinations that the forensic laboratory would provide; and (3) Any other information that the Secretary requires.
