(a) This section does not apply to any controlled hazardous substance that
is:
(1) Used for residential purposes; or
(2) Regulated by the State Department of Agriculture.
(b) Any person who generates, uses, treats, stores, or disposes of a
controlled hazardous substance at any time after 6 months from the date the
Department issues a rule or regulation that identifies the hazardous substance to be
a controlled hazardous substance shall file a report with the Department.
(c) The report required by this section:
(1) Shall be filed within a time set by the Department;
(2) Shall be on the form provided by the Department; and
(3) Shall include the following information about the controlled
hazardous substance, if applicable:
(i) Name of the hazardous substance.

(ii) Volume.
(iii) Quantity.
(iv) Level and kind of radioactivity.
(v) Half-life.
(vi) Manner of management.
(vii) Manner of disposal.
