(a) A qualified organization shall apply for a raffle permit on a form that the county provides. (b) A qualified organization shall disclose the following information on the permit application: (1) the name of the qualified organization, and the names and addresses of its officers and directors; (2) a complete statement of the purposes and objectives of the qualified organization, and the purposes for which the qualified organization will use the proceeds from the raffle; (3) a statement under oath by the president and treasurer, or the chief executive and fiscal officer, of the qualified organization that: (i) an agreement does not exist to divert any of the proceeds of the raffle to another; and (ii) another person will not receive any of the proceeds of the raffle except to further the purpose of the qualified organization; (4) in the case of a raffle of real property, under § 12-106(a) of this article, a copy of the disclosure statement filed with the Secretary of State; and (5) any additional information that the county requires.
