Articles of revival shall include: (1) The name of the corporation at the time the charter was forfeited; (2) The name which the corporation will use after revival, which shall comply with the provisions of this article with respect to corporate names; (3) The address of the principal office of the corporation in this State if different from its principal office in this State at the time the charter was forfeited; (4) The name and address of the resident agent of the corporation; and (5) A statement that the articles of revival are for the purpose of reviving the charter of the corporation.
