An applicant for a license in the county shall include on the application: (1) a statement whether the applicant is a natural-born or naturalized citizen; or (2) if the applicant is not a citizen of the United States: (i) a statement that the applicant's immigration status is in accordance with federal law; and (ii) evidence to verify the immigration status of the applicant.
