Information included in contract for services and waiver of liability

If a written contract for a polygraph examiner's services or a waiver of liability is signed by the
subject of a polygraph examination, the contract or waiver must inform the subject of the procedures
for filing a complaint with the commissioner against the polygraph examiner and contain the name,
mailing address and telephone number of the department. [PL 2013, c. 316, §3 (NEW); PL 2013,
