1. Forms. Application shall be made on forms furnished by the commissioner.

2. Misrepresentation or error. Any license issued through misrepresentation or misstatement
shall be void. Any license issued through error shall be void after notice to the holder.

3. Satisfactory answers. Failure or refusal to satisfactorily answer any question on or about the
application shall be a basis for denying the application.
