1. Forms. Application shall be made on forms furnished by the commissioner. [PL 1977, c. 661, §5 (NEW).] 2. Misrepresentation or error. Any license issued through misrepresentation or misstatement shall be void. Any license issued through error shall be void after notice to the holder. [PL 1977, c. 661, §5 (NEW).] 3.
