Applications for examination

Each applicant for examination shall: (1) Apply to the board on blank forms prepared and furnished by the board. This application shall contain proof under the applicant's oath of the particular qualifications set forth in the application. (2) Furnish to the board two five-by-three inch signed photographs of the applicant, one to accompany the application and one to be returned to the applicant and to be presented to the board when he appears for examination. (3) Pay to the board the fee provided in R.S. 37:375.
