In order to obtain a certificate of approval as a registered equine dentist, the applicant shall comply with the following provisions: (1) Submit an application which shall be sworn to and subscribed before a notary public. (2) Submit evidence that he is a current resident of this state on or before July 1, 1999, and is substantially involved in the care and maintenance of horses in the horse racing industry in Louisiana as defined in the board's rules and regulations. (3) Submit evidence of licensure in good standing issued by the Louisiana Racing Commission on or before July 1, 1995. (4) Pay the fees established by the board, which shall not exceed the following amounts: (a) Original registration fee not to exceed two hundred fifty dollars. (b) Annual renewal of registration fee not to exceed two hundred fifty dollars. (c) Late fee for delinquent registration renewals not to exceed two hundred fifty dollars. (d) Application fee not to exceed one hundred dollars.
