A. An application for court approval of adoptive placement shall be verified and shall contain the following: (1) The name, address, age, occupation, and marital status of the prospective adoptive parents. (2) The expected date of the child's placement. (3) The relationship between the child and the prospective adoptive parents, if any. (4) The name of the child whose placement is requested, if known. B. An application for court approval of adoptive placement shall be filed with the clerk of a court of appropriate venue as authorized by Article 1180.
