No superintendent of a mental institution shall admit a defendant found not guilty by reason of insanity pursuant to Article 654 unless the court furnishes the following information: (1) The defendant's commitment order specifying not guilty by reason of insanity. (2) A copy of the defendant's criminal history record. (3) A police report concerning the charged offense. (4) Victim and witness statements, if any. (5) The name, address, and telephone number of the district attorney who prosecuted the defendant.
