The petition shall set forth a brief statement as to the specific physical or psychological circumstances of the special needs person which form the basis for the petition, and shall include the following information: (1) The name and address of the petiti oner and the petitioner's relationship to the special needs person; (2) The name, age, and address of the special needs person; (3) The name and address of the grantor of the trust, and the grantor's relationship to the special needs person; (4) The name, address, and relationship of the proposed trustee or co -trustees of the special needs trust; (5) The name and address of each reasonably ascertainable interested party; (6) The facts specific to the special needs person and the petitioner to demonstrate to the court that the establishment of a special needs trust for the special needs person is proper under 42 U.S.C. sec. 1396p; and (7) Whether there are any existing liens or claims against the special needs person.
