(1) All applications for certification or licensure shall be made in writing to the board and shall specify the classification of the certificate or license being applied for by the applicant, and shall contain all information required by the board. (2) Applicants for certification or licensure shall: (a) Successfully pass the examination required by the board; and (b) Meet other requirements established by this chapter and administrative regulations of the board. (3) The examination shall be given at times and places within the state as the board shall prescribe; provided, however, the examination shall be given at least two (2) times during each calendar year. Notice of passing or failing the examination shall be provided to each applicant.
