Kentucky Code § KRS 315.4108

List of owners and designated representatives of third-party logistics provider subject to board inspection -- Persons disqualified from being owner or designated representative

(1) Third-party logistics providers shall establish and maintain for board inspection a list of each partner, limited liability company member, and corporate officer and director, including a description of the duties and the qualifications of each. (2) A third-party logistics provider shall not have as an owner or designated representative anyone convicted of a felony for conduct relating to: (a) Providing third-party logistics services involving prescription drugs; (b) A violation of 21 U.S.C. sec. 331(i) or (k); or (c) A violation of 18 U.S.C. sec. 1365 relating to product tampering. (3) A third-party logistics provider shall not have, as an owner or designated representative, anyone who has violated federal or state requirements for third-party logistics provider licensure and presented a threat of serious adverse health consequences or death to humans.
